Otsuka (Philippines) Pharmaceutical, Inc. (OPPI) recognizes its responsibilities under the Republic Act No. 10173 also known as the Data Privacy Act of 2012, with respect to the data it collects, records, organizes, updates, uses, consolidates and destructs. The personal data obtained by the company can only be accessed by its authorized personnel and third parties. OPPI has instituted appropriate organizational, technical and physical security measures to ensure the protection of personal data.

By signing up in this digital platform, I understand and acknowledge that I would be disclosing personal information which would be collected, processed and stored in the OPPI data base system and/or in the member data base system of its authorized third parties, and which shall be used and administered solely by OPPI and its authorized third parties in connection with the implementation and enhancements of this digital resource. 2. The outsourcing of the collection, processing, storage and use of personal information to service providers whether within or outside the Philippines.

The foregoing constitute my express consent under the applicable confidentiality and data privacy law of the Philippines and other jurisdictions, and I agree to hold OPPI and other relevant parties free and harmless from any and all liabilities, claims, damages and suits of whatever kind and nature that may arise in connection with the implementation and compliance with the authorization I confer. The foregoing is without prejudice to my rights to reasonable access to, upon demand, and correction of my personal information, as well as my right to lodge a complaint before the National Privacy Commission, under Section 16 of the Data Privacy Act.

For these purposes, OPPI and its authorized third parties shall store personal information, as defined under Section 3 (g) of the Data Privacy Act, for the duration of my registration and use of this digital resource.

By providing my mobile number and/or email address, I hereby confirm and provide consent for OPPI’s authorized personnel and authorized third parties to send notifications or contact me regarding OPPI’s educational materials for health and wellness, invitations for webinars, information about patient support programs, patient surveys and similar matters.  I understand that I may change my permissions at any time by updating my profile on the profile page. Furthermore, I understand that I can “opt out” of receiving these information and invites by either emailing optout@otsuka.com.ph or clicking the unsubscribe button in the email notifications.

The personal information that we process about you is as follows:
